Description

Sodium nitroprusside is a medication used for the immediate reduction of blood pressure in adult and pediatric patients experiencing hypertensive crises. It is also employed to induce controlled hypotension to minimize bleeding during surgery and to treat acute congestive heart failure.

Indications and Usage

Sodium nitroprusside is indicated for the immediate reduction of blood pressure in adult and pediatric patients during hypertensive crises. Concomitant longer-acting antihypertensive medication should be administered to minimize the duration of treatment with sodium nitroprusside. It is also used to produce controlled hypotension to reduce bleeding during surgery and for the treatment of acute congestive heart failure.

Dosage and Administration

The solution containing 50 mg of Sodium Nitroprusside Injection must be further diluted in 250 to 1000 mL of sterile 5% dextrose injection, depending on the desired concentration. The diluted solution should be protected from light. Sodium nitroprusside should be administered via an infusion pump, preferably a volumetric pump, to ensure precise control. The infusion rate should be started at a very low rate and titrated upward every few minutes until the desired effect is achieved or the maximum recommended infusion rate has been reached. Continuous monitoring of the patient's blood pressure is essential during the infusion. The infusion rate must be guided by the results of invasive hemodynamic monitoring with simultaneous monitoring of urine output when used in the treatment of acute congestive heart failure. Special caution should be used in elderly patients, and titration of the infusion rate must be guided by specific hemodynamic goals tailored to the clinical situation.

Warnings

The principal hazards of Sodium Nitroprusside Injection administration are excessive hypotension and excessive accumulation of cyanide. Small transient excesses in the infusion rate of sodium nitroprusside can result in excessive hypotension, sometimes to levels so low as to compromise the perfusion of vital organs. Nitroprusside-induced hypotension will be self-limited within 1 to 10 minutes after discontinuation of the infusion. Excessive accumulation of cyanide can occur at rates above 2 mcg/kg/min and may lead to serious and even lethal toxic effects.

Side Effects

The most important adverse reactions to sodium nitroprusside are excessive hypotension, methemoglobinemia, and thiocyanate toxicity. Methemoglobinemia and thiocyanate toxicity can occur after prolonged, rapid infusions. Other adverse reactions include cardiovascular effects, dermatologic reactions, gastrointestinal issues, neurologic symptoms, and other miscellaneous effects.

Drug Interactions

The hypotensive effect of sodium nitroprusside is augmented by most other hypotensive drugs, including ganglionic blocking agents, negative inotropic agents, and inhaled anesthetics.
